Short Information : Ayushman Bharat Health Account has released the ABHA Card Digital Health ID Card Registration 2022 notification for the Registration of All India. All eligible and interested candidates can apply online on its Official Website https://healthid.ndhm.gov.in/ Candidates can apply through online mode within Last Date. Detail list of information given below.

Benefits Of ABHA

  • ABHA राष्ट्रीय स्वास्थ्य कार्ड के कई  लाभ हैं, हमने आपकी आसानी के लिए उनमें से प्रत्येक को नीचे सूचीबद्ध किया है।
  • डिजिटल हेल्थ आईडी कार्ड में 4 जरूरी ब्लॉक यूनिक डिजिटल हेल्थ आईडी, प्रोफेशनल रजिस्ट्री, हेल्थ फैसिलिटी रजिस्ट्री और इलेक्ट्रॉनिक हेल्थ रिकॉर्ड शामिल है। 
  • सबसे पहले 4 ब्लॉक के जरिए हेल्थ सर्विस के लिए एक डिजिटल एनवायरमेंट तैयार करना है। 
  • फिर एक इलेक्ट्रॉनिक मेडिकल रिकॉर्ड तैयार होगा जो कि सरकार द्वारा सुझाया गया है। 
  • यह एक प्रकार का किसी मरीज का डिजिटल ब्यौरा है। इसमें मरीज की मेडिकल और ट्रीटमेंट हिस्ट्री दी गई होती है। 
  • आपको बता दें कि आप घर बैठे अपना यूनिक डिजिटल हेल्थ आईडी कार्ड आसानी से बनवा सकते हैं।

ABHA Card Process

  • यूनिक डिजिटल हेल्थ कार्ड बनाने के लिए सबसे पहले आपको नेशनल डिजिटल हेल्थ मिशन की ऑफिशियल वेबसाइट ndhm.gov.in पर जाना पड़ेगा।
  • उसके बाद आपको यूनीक डिजिटल हेल्थ कार्ड का ऑप्शन क्रिएट हेल्थ आईडी नजर आएगा।
  • अब आपको इस विकल्प पर क्लिक करना होगा और उसके बाद कार्ड बनाने का प्रोसेस शुरू होगा।
  • उसके बाद आपसे आधार कार्ड की जानकारी ही ली जाएगी।
  • अब आपको अपना आधार नंबर दर्ज करना होगा और उसके बाद ओटीपी दर्ज करके सत्यापित करना होगा।
  • आप आधार की जानकारी दिए भी हेल्थ कार्ड बनवा सकते हैं।
  • आप सिर्फ अपने मोबाइल नंबर के जरिए हेल्थ कार्ड बनवा सकते हैं।
  • जब आप मोबाइल नंबर देंगे तो उसके बाद आपको ओटीपी के जरिए वेरीफाई करना होगा।
  • अब आपको अपनी प्रोफाइल के लिए तस्वीर, डेट ऑफ बर्थ और एड्रेस समेत कई जानकारियां मुहैया करवानी होंगी।
  • अब आपके सामने एक फॉर्म नजर आएगा, जिसमें आपको जानकारी दर्ज करनी होंगी। जब आप सारी जानकारी दर्ज कर देंगे तो उसके बाद आपके सामने एक हेल्थ आईडी कार्ड नजर आएगा। उसमें आपकी सभी जानकारी जैसे फोटो और एक QR कोड आदि होगा।

Eligibility For ABHA

  • ABHA Ayushman Bharat Health Account Registration 2022 – कंस्ट्रक्शन साइट पर काम करने वाले ,मजदूर, पेंटर, वेल्डर सब्जी बेचने वाले ,राजमिस्त्री ,भार ढोने वाले मजदूर ,रिक्शा चालक, सिक्योरिटी गार्ड ,दुकान पर काम करने वाले लोग, मोची, फेरीवाले, कूड़ा इकट्ठा करने वाले लोग भी Ayushman Bharat Health Yojana के तहत अपना Registration करवा सकते हैं l 

Register Online

  • सबसे पहले आवेदक को आयुष्मान भारत की आधिकारिक वेबसाइट पर जाना है l
  • जब आप ऑफिसर पोर्टल पर जाएंगे, तो होम पेज पर आपको I AM ELIGIBLE का विकल्प दिखाई देगा,इसी विकल्प पर आपको क्लिक करना है l
  • CLICK करने के पश्चात आपके सामने एक New Window खुल जाएगी, जिसमें आपको कई Option दिखाई देंगे l
  • आपको Login के विकल्प पर क्लिक करना है, इसके बाद आपके नंबर पर जो ओटीपी आएगा उसे ओटीपी का आपको सत्यापन करना है l
  • उसके बाद जब आप Ayushman Bharat Health card login करेंगे, तो प्रधानमंत्री आयुष्मान भारत योजना में आपको अपने परिवार की पात्रता को चेक करना होगा l
  • इसके लिए आपको दो विकल्प दिखाई देंगे आपको सबसे पहले अपने राज्य का चुनाव करना है l
  • इसके बाद आपको दूसरे विकल्प में तीन कैटेगरी दिखाई देंगी जिसमें से आपको नाम, राशन कार्ड, मोबाइल नंबर आदि के द्वारा परिवार की पात्रता सत्यापन करनी है l आपको किसी भी विकल्प पर क्लिक करके सबमिट कर देना है l
  • यदि किसी हॉस्पिटल से आप का उपचार चल रहा है, तो उसका प्रमाण पत्र आपको लगाना होगा l
  • इस प्रकार से रजिस्ट्रेशन प्रक्रिया पूरी हो जाएगी I

Register Offline

  • ऑफलाइन आवेदन करने के लिए सबसे पहले आपको नजदीकी जन सेवा केंद्र पर जाना है l
  • नजदीकी जन सेवा केंद्र पर आपको अपने सभी मूल दस्तावेजों की फोटो कॉपी जमा करवानी है l
  • इसके बाद जैसे ही जन सेवा केंद्र के एजेंट द्वारा दस्तावेज़ सत्यापन करवा लिए जाएंगे, तो रजिस्ट्रेशन प्रक्रिया हो जाएगीl
  • इसके बाद 15 से 20 दिनों बाद जन सेवा केंद्र के द्वारा आपको Ayushman Bharat Golden Card (आयुष्मान भारत का गोल्डन कार्ड) दे दिया जाएगा l इस प्रकार से आप का रजिस्ट्रेशन हो जाएगा l

  • Step 3: Enter Your Mobile Number and Captcha
  • Step 4: Send OTP Button and Enter OTP (One Time Password)

  • Step 6: Upload Required Documents
  • Step 7: Check All details carefully and Final Submit the Application Form.
  • Step 8: Print Digital Health ID Card
